Community Connection’s Family Caregiver Support Program acknowledges and encourages the vital role caregivers play in our country’s home and community-based services through support in a coordinated and responsive manner by taking care of those who take care of others. There is no charge for these services.

What kind of services are included in the Family Caregiver Support?

Services are intended to complement, not replace, the care provided by caregivers.

Services to support the needs of caregivers on a limited basis may include:

  • Respite care, including Adult Day Care, to provide a caregiver with a temporary relief from the demands of daily caregiving
  • Information about services and assistance accessing services
  • Minor home modifications and/or adaptive aids
  • Training for caregivers, such as the Powerful Tools workshop

What are the eligibility guidelines?

The Family Caregiver Support Program may be able to provide assistance to you if you are caring for someone who is:

  • At least age 60
  • Any age and has Alzheimer’s disease or other dementia
  • A child under the age of 18 and you are a non-parent relative 55 years of age or older
  • An adult with a disability and you are a non-parent relative 55 years of age or older
